Interview Sample
Hello—let’s begin the case interview. Could you tell me about a recent challenging case that stood out to you?
Yes. I had a case involving a 68-year-old man who presented to the ED with symptoms concerning for stroke. The presentation was atypical, which made the diagnosis challenging.
You mentioned the initial presentation was atypical—what features, specifically, made the diagnosis difficult?
He mainly complained of dizziness rather than focal weakness, so it initially seemed more consistent with BPPV than stroke. We ultimately confirmed a cerebellar infarct only after obtaining an MRI.
So the final diagnosis was a cerebellar infarct. How did the treatment course go, and what was the patient’s outcome?
We started antithrombotic therapy promptly, and fortunately he was discharged without significant residual deficits.
Looking back, what was the biggest pain point in this case—either from a systems perspective or in terms of patient care?
The main issue was the delay in obtaining neuroimaging during the initial ED evaluation. Because his symptoms were nonspecific, he was triaged as a lower priority, and we nearly ended up with a significant diagnostic delay.
